Dive Cancellation - Active Divers Association

Please complete this form for each dive you wish to cancel. You can cancel for multiple divers under the same reservation at time of purchase by including their full names separated by commas in the field below. If this cancellation is within 24 hours of the dive please also contact the Safety Officer directly to confirm the cancellation. You must submit a cancellation form for each dive you have booked in order to receive a refund for the amount you paid for the dive, less a $10 processing fee. Dives will not be refunded until a cancellation form is submitted.
